REPEAT PRESCRIPTION REQUEST 

We are unable to accept requests for repeat prescriptions over the phone.  This policy is strictly adhered to and is in place for your safety.

We ask you to attend your doctor for review of your medications at least once every year. For some patients it may be necessary to attend more often.

We aim to have prescription requests processed within 1-2 days.
You need to check directly with your pharmacy yourself for when your medication will be ready for collection.

Thank you for choosing us! We're delighted to welcome you to Grange road family practice. Please fill in the form at the below link and send in to us through email, post, carrier pigeon - your choice! Please give 48hrs for your forms to be processed before calling. 

TRANSFER YOUR FILES

If you would like to request your files from your previous GP - please fill in the attached and send on to your old GP or back to us and we will be happy to do it for you.

TRANSFER YOUR GMS/DVC

If you currently have a GMS or DVC with another GP but wish to transfer to our practice. Please complete & sign the attached form and return to us for processing.

TRAVEL VACCINE FORM

We require you to complete a travel vaccine consultation checklist prior to your travel vaccination appointment. Please complete at least one month prior to travelling and email to reception for review.

TRIAGE FORM

If the GP is fully booked, you might be asked to complete a triage form by one of our reception team. Please fill in as much detail as possible in order for our GP to triage effectively. This form should only be used if an urgent assessment is needed and cannot wait until the next available appointment. Please email the form to reception@grangeroadfamilypractice.com

REPEAT PRESCRIPTION FORM

Beginning 1st August 2023, this practice will operate a policy of accepting repeat prescription requests in written form only. Our admin team are no longer permitted to accept verbal requests. You must fill in a prescription ordering form in full and email, post or hand deliver to the practice. Forms are available from reception and by clicking the link below.
